Credit Card Authorization Form

The undersigned hereby authorizes the guarantee of the minimum spend as agreed upon between the Client and the venue listed above. If the reservation is cancelled or the number of guests decreases and the Client notifies the Venue with more than 48 hours notice, there will be no penalty. If the Client calls ahead to cancel the reservation within 48 hours of the event, he or she will be subject to a $25 per person cancellation fee. If the Client does not notify us ahead of time, to decrease the number of guests or cancel the reservation, the client is responsible for payment for the full above, agreed upon per person prix-fixe or minimum spend. If the party exceeds the number of guests agreed upon the Client will be charged for each additional guest (at the per person rate agreed upon for the original party size). Tax (8.875%) and gratuity (20%) or admin (25%) will be added to all charges.

• No decorations (except table top pieces) and no balloons, confetti, banners, etc.
• There is an additional $3 per plate cake cutting fee for any outside desserts.

All tables are held for 20 minutes from the start of your reservation. If a member of your group is not present by this time, your table will be given to the next party. If a cancellation is not confirmed via email you will be subject to the above charges.

By signing this form, the venue is authorized to charge this credit card in the event that the minimum spend is not achieved or the above noted cancellation/no show policy is not complied with. Otherwise this card is only to be used to hold the reservation and the final tab can be split between four forms of payment.
